Enrollment officially closed as of November 2013 and observational follow-up (i.e. questionnaire based) remains ongoing. Participants are asked to reply to a brief health questionnaire from the Clinical Coordinating Center at Brigham and Women's Hospital in Boston, Massachusetts, every 6 months.
