If you are mailing the follow-up questionnaire, please remember to return both pages for processing purposes. In addition, if your contact information has changed, please be sure to include the new information on the back/second page of the full follow-up questionnaire OR complete the "Update Contact Information" tab on the left hand side of the screen. Since this is a long-term observational follow-up study, it is very important that we maintain regular contact with you. It is common that participants often move and forget to notify us of their new address. Therefore, we will use your personal contacts to reconnect with you. As a reminder, please be sure to inform your contacts that you are enrolled in the PRE-DETERMINE Study so they are aware that we may need to contact them in the future.
